Youth Social Prescribing is a voluntary service for young people aged 11–25 and their families. It offers a whole-person approach, providing non-medical support to address the social and practical things that can affect your health and wellbeing.

We may be able to help with

  • Feeling lonely or disconnected
  • Confidence, self-esteem, motivation or routines
  • Low-level stress, worry or low mood when there is no immediate crisis
  • Sleep, physical activity and healthier routines
  • Finding groups, hobbies, volunteering or positive activities
  • Practical barriers such as transport, appointments, money worries or accessing services
  • Staying connected to strengths and community support alongside other professional help

What happens next

  • We check your details and make sure you are happy for us to contact you.

  • You agree a simple, personalised plan with manageable next steps.
  • Support can include up to 12 one-to-one sessions and links to activities, groups or practical help.

How to Access

When another pathway might be needed

Youth Social Prescribing is designed to support young people with social, emotional and practical needs that affect wellbeing. It may not be the right first step where a young person needs urgent or specialist support, such as clinical intervention, crisis support, or immediate safeguarding action.

In these situations, it is important that the right support is in place first. Once immediate or specialist needs are being responded to, we may be able to work alongside this support to help with social needs, confidence, routines, connections and practical next steps.

In an emergency, call 999.
